Care Managers play an important role in The Florida Pooled Trust.

     When a Beneficiary joins the Trust, their joinder agreement must include an individualized care plan. The Care Manager prepares this individualized care plan or reviews an existing plan, if available. The care plan goes into the Beneficiary’s file and becomes part of the information used by the Trustee when reviewing future requests for distributions.

     Care Manager involvement helps to ensure that distributions will meet the Beneficiary’s goals and objectives as identified in the original care plan. Care Manager involvement also can help identify needs that are not currently identified or that have arisen because of changes in the Beneficiary’s circumstances.

     Because the Trust is structured to encourage maximum participation and to provide maximum value, the annual administrative fee does not include fees for Care Managers. Beneficiaries only pay for Care Managers if, and when, they need them. This arrangement prevents Beneficiaries who use Care Managers infrequently from subsidizing services for Beneficiaries who use Care Managers on a more frequent basis.
